Procedure working with a third party
With each arrival or departure, the contractor/third party must first report to the reception desk: Kortemarkstraat 86 - 8810 Lichtervelde.
There he/she will be registered in the logbook and will receive a badge (this will be returned to reception desk on departure).
General information
1. The requirements set out in this document must be imposed by the contractor/supplier on its personnel and subcontractors (in accordance with Section 1 of Chapter IV of the Act of 4 August 1996 on the welfare of employees in the performance of their work) The contractor/supplier guarantees that these requirements are complied with, and that their subcontractors in turn impose these requirements on their personnel and other contractors.
2. If the appointed contractor has work carried out by third parties, this must be reported prior to the execution of the work and approved in writing by OptimaT.
3. Work that must be carried out outside normal hours or during days off (weekends, leave, etc.) can only be carried out if someone from OptimaT can be present. These works must be requested in good time. If not, access to the workplace will not be granted.
4. The contractor permanently complies with the provisions of : Wet Welzijn, ARAB,CODEX,AREI, VLAREM, VAREBO, VLAREMA, social legislation, fiscal legislation and all applicable laws and regulations.
To this end, he gives the necessary training & instructions to his employees. The contractor bears the final responsibility.
5. At least one Flemish-speaking superior of the contractor shall be permanently present.
6. It is forbidden to bring alcoholic beverages or drugs into our company.
7. Cameras are not allowed.
8. Only the places related to the assignment may be entered, and this via the proposed route.
9. Third parties subcontracted by the contractor are under the supervision and responsibility of the contractor and must meet all the conditions mentioned in this form. To this end, the contractor shall hand over a copy of these regulations to the subcontractor(s), who in turn shall sign them and return them to Andy Vandekerckhove (OptimaT prevention advisor).
10. In the event of serious infringements or urgent reasons, OptimaT may, on its own initiative and at the expense of the contractor, take corrective measures (e.g. provision of personal protection equipment, immediate cessation of the relevant works,...).
11. If the operator is required to have a certain permit for certain work, this must be submitted on simple request.
Fire - Evacuation
12. FIRE ACCESSORIES: for works with open fire, white flames or heat points (e.g. welding, cutting fires, soldering, stripping, grinding, burning off paint or varnish, thawing, places of roofing, ...) a fire permit must be applied for with Andy Vandekerckhove (Prevention Advisor), this before the start of the works, this with the OptimaT prevention service.
No exception is made in the case of unforeseen and urgent works.
13. There is a general smoking ban within OptimaT. Smoking is only permitted outside the buildings at the places provided for that purpose. GRONDPLAN available at the OptimaT reception.
14. All gates, doors, passageways, electrical cabinets and fire extinguishing equipment must be kept free at all times.
15. In the event of an alarm (either siren or verbal call), the contractor must immediately leave the buildings and go to the assembly point. (near the reception) there he will take his place at the level of the Black lacquered sign. The buildings may only be re-entered after approval from the management of OptimaT. GRONDPLAN available at the reception of OptimaT.
16. You notice a fire:
a. Press a fire alarm button located near you.
b. Notify the prevention service: Andy (0486/960 443) and state:
i. Who you are
ii. Where the fire occurs
iii. What is on fire
c. Prevent expansion :
i. Try to make use of the extinguishing agents available (GRONDPLAN
available at the OptimaT reception)
d. Do not use the elevators
e. Lead yourself and those present to safety
Machines - Tools
17.Use of material and tools, property of OptimaT, is only permitted with prior written permission.
18. Forklift trucks, stackers and hoisting equipment (overhead cranes, hoists), owned by OptimaT, may only be used subject to written permission. The contractor shall ensure that his employees receive the necessary training to operate such equipment.
19. A copy of the last inspection report must be submitted on simple request for all equipment brought by the contractor for which a statutory inspection applies.
20. Obligation to report damage: In the event of damage to factory property or installations, this must be reported immediately to the prevention service.
Protective equipment
21. In certain departments of OptimaT, it is compulsory for everyone to wear safety glasses and/or hearing protection. This also applies to the employees of contractors. Those who do not wear glasses and/or hearing protection will be denied access to the workplace.
22.The contractor's employees must have adequate personal safety equipment and use/wear it correctly.
23. When working in the SAR/Cleanroom the hygiene rules must be followed.
Waste
24. After completion of the work and/or at the end of the day's work, the work area must be left clean and tidy. All waste must be removed from our grounds by the contractor himself.
25. In the event of a calamity (spillage of soil polluting products), the prevention service must be informed immediately. All costs resulting from such a calamity shall be borne by the contractor.
First aid and accidents
26. The contractor shall provide the necessary first aid resources himself.
27. Every accident or incident must be reported to Andy Vandekerckhove (Prevention Advisor OptimaT).
